Registry Information
North Carolina does list Nurse Aides by reciprocity.
Nurse Aides are responsible for ensuring their name and contact information are current in the registry. To
update information, submit the Registry Change Request Form.

Renewal Process:
- Registry listings must be renewed every 24 months.
- Nurse Aides are responsible for knowing when their listing expires. The Division of Health Service Regulation
does not send renewal notifications.
- To qualify for renewal, the Nurse Aide must meet the following requirements. Nurse Aides who do not meet these
requirements must retrain and retest. Qualified work can occur in a Nursing Home or other health care setting.

- Job duties performed were in the capacity of a Nurse Aide.
- Job duties performed were for pay.
- Job duties performed were delegated and supervised by a Registered Nurse.
- Job duties were performed for at least eight (8) hours during the 24 months associated with their registry
listing period.
- A registry listing cannot be renewed if the Nurse Aide has a substantiated finding of abuse, neglect, exploitation, or
misappropriation of resident or patient property and employed in a Nursing Home facility.
- Two registry forms are required to renew a listing:
- The expiration date is based on the last date of qualified work reported by the Registered Nurse in
the Online Employment Verification Form.
Expiration dates are extended to the last day of the month.
